Registration Information: Check with your local county office for registration information.
State 4-H Conference is held every July on the NMSU Campus. The Conference consists of contests, workshops, evening sessions, awards ceremony, and the election of new State Officers.
Those eligible to attend State Conference are senior 4-H'ers who participate in contests, workshops, and sessions; voting delegates and officer candidates. For registration information please check with your County Extension Agent. Agents: To register your counties for State Conference please go to the 4HOnline Web Page. Registration information will only be accepted by the County Extension Agent.

Youth Combined Liability Forms (DOC)
This combined form is REQUIRED for each and every 4-H event. This is a new form which replaces the former Medical Release, Code of Conduct, and Photo Release forms.

Adult Combined Liability Forms (DOC)
This combined form is REQUIRED for each and every 4-H event. This is a new form which replaces the former Medical Release, Code of Conduct, and Photo Release forms.

Consent Form for Overnight Lodging with Non-Custodial Adult
Only needed if youth will be rooming with an adult/chaperone other than a parent.
