Procedures

  1. Pick-up a Survey Review/Application for Permission to Use Human Subjects in Research packet at the Agricultural Biometric Service office in GT 100.
  2. Read the instructions.
  3. Type or word-process the upper portion of the College Survey Review Committee Recommendation form. The form is also available electronically under the Agricultural Biometric Service section of the Agricultural and Extension Education Department website (http://cahe.nmsu.edu/academics/axed). The chair of the committee will fill out the lower portion.
  4. Type or word-process the Survey Design Summary. The form is also available electronically under the Agricultural Biometric Service section of the Agricultural and Extension Education Department website (http://cahe.nmsu.edu/academics/axed).
  5. Complete the Application for Permission to Use Human Subjects in Research form and other documentation require by the Institutional Review Board (IRB). Be sure to get required signatures on all appropriate forms.
  6. From the packet return to the Agricultural Biometric Service Office (GT 100):
    1. College Survey Review Committee Recommendation form.
    2. The Survey Design Summary.
    3. Application for Permission to Use Human Subjects in Research form and other documentation required by the IRB (with required signatures).
    4. A copy of your survey.
  7. At this time your packet will be logged in and a copy routed to two of the committee members including at least one statistician. The committee members will make their comments and recommendations. The chair of the committee will contact the researcher if changes need to be made. If there are no changes or when the survey is acceptable to the committee, the chair will sign off on the survey and the packet will be routed to the Dean for his/her signature.
  8. The packet is picked up from the Dean’s office by the Agricultural Biometric Service and a copy is made before routing it to the Vice Provost for Research who will provide it to the IRB. The researcher, upon request, can hand-deliver their packet to the Vice Provost for Research.
  9. NIH surveys should follow NIH survey review policies.
